Opioid Warn-Me Label Order Form

The U.S. opioid crisis is taking too many lives, and NSCN wants to make you, your family and your community safer. We encourage you to use these warn-me labels to limit the risk from opioids. NSCN members can order these resources at no cost!

Warn-Me Label Card - Self-stick label is sized to fit on insurance and prescription cards without covering up important information. Many people don’t know that commonly prescribed drugs such as Vicodin and Percocet are opioids. The label lets medical professionals know that patients want to discuss the risks of prescribed opioids and whether there are safer, effective alternatives. Four labels per card to share with family and friends.

Warn-Me Label Sheet - Nearly half of companies have suffered a negative business impact from opioid use by employees. These labels let medical professionals know that patients want to discuss the risks of prescribed opioids and whether there are safer, effective alternatives. Each sheet has 75 self-stick labels; order enough sheets so you can distribute labels to all your employees and their family members.
