Everyone makes mistakes. Offered at the discretion of a prosecuting attorney, our voluntary Diversion program offers a one-time opportunity for individuals charged with criminal offenses to avoid having a criminal conviction on their record.

  • Community service is required to be completed every month as part of Diversion Program requirements. See your Program Agreement. You may complete more than the required hours per month. You may complete hours at different agencies on this list. Please make sure a Diversion Program Community Service card is filled out completely and clearly for EACH AGENCY where community service is performed. Cards must be turned in every month.
  • You MUST have pre-approval from the Diversion Director or Supervisor before volunteering at an agency that is not on this list to ensure those hours will be accepted.

Instructions for Submitting Community Service Hours

How do I submit/prove my completed community service hours?

  • Participants must complete a Service Log Sheet for each organization in which he/she volunteers.
  • A signature from a contact at the agency is required on each form.
  • Completed forms must be submitted by the monthly deadline located on your requirement checklist.
  • Participants may complete more than their monthly minimum; any additional hours will carry over to the following month.
  • The completed form(s) can be submitted below, faxed to: 402-896-6331, or dropped off to our office in person at 11620 M Circle, east/upper side of the building.
